The CoxHealth Foundation provides a program by which patients being treated at CoxHealth can apply for financial support.
To be considered the following is needed:
Applications are considered incomplete if the application is not completely filled out and the required documents including a proof of income and a healthcare referral, are not included along with your explanation of need. Please give as complete an explanation as possible to assist us in finding the best match to funding available for your request.
